How Choctaw County Death Records Work

All death records in Oklahoma are filed at the state level. When someone dies in Choctaw County, the funeral director submits the death record to the Oklahoma State Department of Health. The Office of Vital Records stores and issues certified copies. This is required by Title 63 O.S. Section 1-323. The Choctaw County Clerk at 300 E. Each certified copy costs $15. The fee is not refundable. You can order in person, by mail, or online. Walk-in service is available at the OSDH office at 1000 NE 10th Street, Oklahoma City. It takes about one hour. Will-call hours are 12:00 PM to 4:45 PM on weekdays. The Tulsa office at 5051 S 129th East Ave and the McAlester location at 1400 East College Ave also accept requests. McAlester is the closest walk-in office to Choctaw County.

For mail orders, send your application to PO Box 53551, Oklahoma City, OK 73152. Include a check or money order for $15 payable to OSDH, a signed request, and a copy of your photo ID. Mail processing takes about four weeks.

Choctaw County Health Department

The Choctaw County Health Department is located at 103 S 4th St in Hugo. It is a small office with about three employees. The health department offers basic public health services like immunizations and screenings. It does not issue death certificates. Staff can tell you how to contact the state office, but the actual records come from Oklahoma City.

Who Is Eligible for Choctaw County Death Records
