Logan County Death Certificate Process

Death certificates in Logan County follow the same rules as the rest of Oklahoma. The funeral director files the certificate with the state after a death. The Oklahoma State Department of Health keeps it on file. You cannot get a certified copy at the Logan County courthouse. The county clerk does not handle death records. There is also a Tulsa office at 5051 S. 129th East Ave and a satellite in McAlester at 1400 East College Avenue. Will call pickup runs from 12:00 to 4:45 PM. Same day service is no longer available. The city of Edmond borders Logan County and many residents there also use the Oklahoma City office for vital records requests.

Mail requests go to Vital Records Service, Oklahoma State Department of Health, PO Box 53551, Oklahoma City, OK 73152. Include a completed form, a photocopy of your ID, and a check or money order for $15 per copy. Do not send cash by mail.
